Qu’est-ce qu’un\u00a0traumatisme cr\u00e2nien<\/a>?<\/p>\n

\"\"<\/p>\n

Sur le plan clinique, il existe\u00a0trois cat\u00e9gories principales de traumatismes cr\u00e2niens\u00a0:<\/h3>\n
    \n
  1. L\u00e9gers(sans perte de connaissance et sans fracture de cr\u00e2ne),<\/li>\n<\/ol>\n

    2. Moyens(avec une perte de connaissance initiale exc\u00e9dant quelques minutes ou avec fractures de cr\u00e2ne)<\/p>\n

    3. Graves(avec coma d\u2019embl\u00e9e sans ou avec fractures de cr\u00e2ne associ\u00e9es).<\/p>\n

    La\u00a0Glasgow coma scale (GCS)<\/strong>\u00a0a \u00e9t\u00e9 \u00e9labor\u00e9e par d\u00e9velopp\u00e9e par G. Teasdale et B. Jennet \u00e0 l\u2019institut de neurologie de Glasgow en Ecosse et date de 1974.<\/p>\n

    Cette \u00e9chelle de gravit\u00e9 initiale permet de distinguer les diff\u00e9rents niveaux de gravit\u00e9 du traumatisme selon l\u2019existence et la profondeur d\u2019un coma initial appr\u00e9ci\u00e9 par le\u00a0score de coma de Glasgow<\/strong>\u00a0(GLASGOW COMA SCALE).<\/p>\n

    <\/h3>\n

    Cette \u00e9chelle se d\u00e9compose de la mani\u00e8re suivante :<\/h3>\n
      \n
    • Traumatisme cr\u00e2nien grave<\/strong>\u00a0: 3 \u00e0 8<\/li>\n
    • Traumatisme cr\u00e2nien mod\u00e9r\u00e9<\/strong>\u00a0: 9 \u00e0 12<\/li>\n
    • Traumatisme cr\u00e2nien l\u00e9ger<\/strong>\u00a0: 13 \u00e0 15<\/li>\n<\/ul>\n

      C\u2019est en additionnant les scores correspondant aux trois cat\u00e9gories suivantes que l\u2019on fixe le GCS du patient.<\/p>\n

      Ainsi cet\u00a0outil d\u2019\u00e9valuation<\/strong>\u00a0\u00e0 port\u00e9e imm\u00e9diate permet d\u2019\u00e9tablir une\u00a0classification de la victime traumatis\u00e9e cr\u00e2nienne<\/strong>\u00a0ce qui a son importance car sa prise en charge sera diff\u00e9rente.<\/p>\n

      Cette \u00e9chelle est d\u2019ailleurs largement utilis\u00e9e. Il faut cependant noter qu\u2019en 1985 a \u00e9t\u00e9 mise au point une \u00e9chelle un peu moins utilis\u00e9e mais s\u2019attachant particuli\u00e8rement \u00e0 l\u2019\u00e9valuation d\u2019un traumatisme cr\u00e2nien grave. Il s\u2019agit de l\u2019\u00e9chelle de Glasgow Li\u00e8ge invent\u00e9e par JD Born and all visant \u00e0 am\u00e9liorer la GCS.<\/p>\n

      Celle ci est\u00a0bas\u00e9e sur les r\u00e9flexes du tronc c\u00e9r\u00e9bral<\/strong>\u00a0comme le r\u00e9flexe fronto orbiculaire, le r\u00e9flexe oculoc\u00e9phalique, ou oculovestibulaire vertical, r\u00e9flexe photomoteur, r\u00e9flexe enc\u00e9phalique ou oculovestibulaire horizontal, r\u00e9flexe oculocardiaque.<\/p>\n

      Le GLS \u00e9tend la sensibilit\u00e9 du GCS dans les phases de coma profond, mais n\u2019est pas plus sensible pour \u00e9valuer des patients en \u00e9tat v\u00e9g\u00e9tatif ou en \u00e9tat de conscience minimale.<\/p>\n

      La Glasgow coma scale est la premi\u00e8re \u00e9chelle utilis\u00e9e afin d\u2019\u00e9valuer la gravit\u00e9 du traumatisme cr\u00e2nien dans son stade initial.<\/em><\/p>\n<\/div>\n
